Emerging Modes of Medicine
Medicine is not converging on a single future. It is fragmenting into a constellation of modes, and understanding that constellation is the precondition for visioning the physician who must work within it.
Recommendation
Recommendation. Adopt a proactive, systematic approach to disruptive change across the modes of medicine, positioning the physician as a key player inside each rather than a downstream recipient of paradigms chosen elsewhere. Afford the physician normative governing influence within the learning health system, with authority to rule inferences out of bounds, to declare unmeasured goods protected, and to require that a proposed change to practice be justified against something other than the metric that proposed it. Build per-case reliance feedback as core clinical infrastructure. Use three levers that decide where a capability lands: reimbursement category, regulatory classification, and adjacent workflow ownership, years in advance. Strive towards social and global solutions to the exposomics disconnect.
