Chapter X
The Global Physician
Complementing the physician-patient I-Thou is the physician-community commitment. The two are intricately connected, and neither is optional.
Recommendation
Recommendation. Integrate global health studies and experiences into physician formation as a required element rather than an optional add-on, recognizing that much of it need not even require boarding a plane, since case-based sessions across health systems, joint research, shared journal clubs, and cross-national mortality reviews all operate at distance. Specify what that formation must produce as transferable competencies: reading a population as fluently as a patient, judging whether a tool validated elsewhere applies here, and working as a guest in a system not one's own. Hold algorithmic procurement and health policy decisions to the standards medicine applies to more immediate public health interventions, requiring a stated population effect, transparency about who is missed, and monitoring after deployment. Embed equitable access requirements in global biomedical innovation frameworks at the point of invention, rather than after a product exists and its price is set. Ensure that physicians are at the table when it comes to global health governance, security, and ethics.

- 05Mandatory Global Training ExperiencesGrasp of global health issues and engagement in global health efforts should be ingrained in physician education in a structured, integral way, not offered as an optional add-on.Moonshots
