The Physician in 2050

Chapter V

Personalizing and Integrating Medicine

The ambition is N-of-1 medicine: ultra-personalization of diagnostic, therapeutic, and preemptive solutions, together with the human touch. Machine intelligence brings us closer, but a gap will persist that delineates the human domain.

Recommendation

Recommendation. Reorganize medicine around preemptive health, built on N-of-1 reach, continuous monitoring, and long-horizon stewardship. Support clinical research that can narrow the N-of-1 gap: EHR data mining and complex system decoding of pathophysiologic mechanism. Design explicitly for the interpretive and communicative work only a human can do. Ask each patient directly for what they want a physician to remain answerable, rather than inferring it from the category into which they fall. Commission the research to better understand what patients will want once machines match physician performance in selected areas, because preferences take longer to decipher than systems take to accommodate them. Hold preemptive health interventions to a strict standard: outcomes rather than surrogates; net benefit balanced against the harms wrought by patient labeling; and expected rate of overdiagnosis. Settle by consortium the workflow, provenance, and liability rules for clinical digital twins, on the principle that prediction is an input, but the decision belongs to the physician. Create a clearing in the forest for holistic-integrative medicine, with an evidentiary basis, and reorient systems towards lifetime health trajectories.

  3. 03From Repair to Preemptive MedicinePersonalization changes the timing of care as well as its precision.
  4. 04On the Road to Holistic/Integrative MedicineN-of-1 reasoning widens naturally into holistic medicine, a term that should be rescued from its history.1 definition
  5. 05Rethinking the Evidence in Evidence-BasedEvidence-based medicine is conventionally defined as the conscientious, explicit, and judicious use of current best evidence in making decisions about the care of individual patients, integrating individual clinical expertise with the best available external evidence and the patient's values.Moonshots
  6. 06The Patient of 2050A report that redefines the physician owes an account of the patient, and this one has so far described the patient chiefly as a biological particular: a genome, an exposome, a stream of measurements, a twin. That is one kind of particularity.
